Please do not try. A double car door weighs well over a hundred kilograms and the spring is what carries that weight — with it broken, the door is effectively dead weight held by cables that were never meant to take it. Leave it down, do not run the opener, and park elsewhere. Replacing a spring is a same-day repair for us and an emergency room visit for a lot of people who try it themselves.

Replacing one means paying the service call twice and running an unbalanced door in between, which is hard on the opener.
Springs are rated in cycles — one open and one close is a cycle — and a standard spring is often rated around ten thousand, which is roughly seven to twelve years for an average family. A high-cycle spring costs a modest amount more and lasts substantially longer, which is worth it if your garage door is your main door.
Almost never. Noise is usually worn rollers, dry hinges, loose hardware or bearings on the way out, and a tune-up with new nylon rollers transforms how a door sounds — particularly with a bedroom above the garage. Replace the door when the panels are damaged or rusted through, or when you want insulation, not because it is noisy.
If the garage is attached, heated, used as a workshop, or has a room above it, yes. The insulation also makes the door quieter and considerably more rigid, which means it holds up better over the years. For a detached, unheated garage used for storage, the payback is much weaker and a non-insulated door is a reasonable choice.
